I provided specialized expertise for the team in a field of social work (social care assessment, mental capacity assessments, best interest meetings), I also worked closely with families of my clients in order to provide the best possible outcomes, my client group was people over 65 with a diagnosis of mental health difficulties, including dementia.

I was assessing people’s needs in a fast pace service covering both routine appointments (10 working days) and urgent assessments (within the same day, often in 2-3 hours from the referral). I delivered short-term psychological interventions (up to 4 sessions) for people with complex needs and high risk to ensure safe transition to Community Teams. I supported and supervised junior staff to ensure quality of provided service.

I was screening and triaging referrals to mental health services for adults of any age. I worked closely with referrers to ensure the referral being passed to an appropriate services. I regularly contacted patients to discuss available support and negotiate access to the best possible service for the client. Occasionally, in complex cases, I met with the patient and performed an assessment. I used AMIGOS and MICARE software on daily basis. 

 

07/2013- 03/2016 Sexual Assault Referral Centre, Manchester, Crisis Worker

 

I provided emotional support for adults and children who were sexually assaulted. We worked as an acute unit and therefore I met with people when they are few hours after the event and in shock/crisis state. I supported clients in decision making process and throughout medical examination, should the client choose to proceed. I predominantly took care of ensuring that the client’s immediate needs are met and she/he left the service equipped in relevant information and support in community. I also supported service users in resolving crisis via 24/7 helpline.

 

09/2012- 10/2013  Early Intervention Team in Psychosis, Manchester, Care co-ordinator

 

I co-ordinated the care (up to 3 years time) of case load of clients aged 14-35 experiencing first episode of psychosis. I worked within CPA, assessed client’s needs and coordinated delivery of the support plans. I worked independently and as a team member. I worked in close relation with other professionals and other organizations, including charities, statutory children and family services, CAMHS and police to support both client and their families in understanding illness and treatment. I negotiated best, recovery-focused care for client. I used various NICE guidance recommended psychological and psychosocial interventions with clients and their families. I participated in safeguarding investigations and service improvement meetings.

I worked to promote independent living of my clients and to provide companionship and support. I worked with service users in order to reach service user's goals. I risk assessed on daily basis. I was providing support with daily living, promoting healthy living and worked with service users to identify early signs of relapse. I supported service users to take active part in their care and treatment and negotiating support package. I was liaising with families, carers or significant people when appropriate and co-operated with other professionals. I was working as a part of team in order to provide best possible help to service users. I was collecting and verifying information about relevant to service local organizations and institutions. I was facilitating psycho-education group for alcohol addicted and providing motivational therapy, together with telephone and internet support as a part of crisis helpline. I worked with families and children and provide psychological interventions focused on family dynamics and client’s personal development. I facilitated negotiations process within families. I was delegated to supervise and team lead volunteers and junior members of staff. I worker with both, teenagers and adults regardless their age.
